Disclaimer 1: Qoros Clear Lake Surgery Center offers patients 3 bundles listed by the Novitas Solutions pricefinder. These include Cardiac Cath-Diagnostic, Coronary Angioplasty, and Coronary Angioplasty with placement of drug eluting stent.

Disclaimer 2: We do not contract with Anesthesia or pathology groups at this time and no billing will be expected from these groups. Patients should contact the Anesthesia or pathology groups to get an estimate for their services.

The actual costs will be based on the services provided to the patient and those costs may be more or less than the typical costs cited for the basic services listed on the bundles published on the Novitas Solutions pricefinder website.

Patients may request a personalized estimate of costs for services upon submission of the CPT for services their physician anticipates performing while a patient at our facility. This information may be submitted in writing to the facility for response by the Administrative team.

The facility does not offer financial assistance and while a Medicare participating facility, does not yet participate in the Texas Medicaid program.

The facility will attempt to set up mutually agreeable payment plans for patients that will allow the out-of-pocket costs to be paid over a number of months interest-free.

The facility will send out 3 patient statements and as long as the agreed-upon monthly payment is made, further statements will be sent. If payments are not made on a regular monthly basis, patients will be referred to one of the collection agencies utilized by the facility for delinquent accounts.

Patients are responsible for contacting the physician who will perform services on them to determine if the provider participates with their medical insurance carrier as individual physicians determine what plans they participate in and not the facility.
